I understand that attendance of a dog obedience training class is not without risk to myself,
members of my family or guests who may attend, or my dog, because some of the dogs to
which we will be exposed may be difficult to control and may be the cause of injury even when
handled with the greatest amount of care, and because dog training is a sport which by its
nature may result in physical injury.
I hereby waive and release The Pet Coach and Donna Ball, her employees and assigns, from
any and all liability of any nature, for injury or damage which I or my dog may suffer, including
specifically but not limited to, any injury or damage resulting from the action of any dog, and I
expressly assume the risk of any such damage or injury while attending any training session or
other function associated with training, or while on the training grounds or the surrounding
area thereto.
In consideration of and as inducement to the acceptance of my application for training
membership in this training class, I hereby agree to indemnify and hold harmless The Pet
Coach and Donna Ball, her employees and assigns, from any and all claims, or claims by any
member of my family or any other person accompanying me to any training session or function
associated with training or while on the grounds or the surrounding area thereto as a result of
any injury or damage to said person or persons in whatsoever manner it may occur.

Special Instructions
PLEASE READ BEFORE SIGNING UP FOR THIS CLASS
Sign and return with your application form
We want your experience at The Pet Coach to be safe and enjoyable for both you and your
pet. In order to make your time with us as rewarding as possible, we have a few simple rules.
Your signature below indicates that you have read and agree to abide by these rules. If you
cannot do so, you will be asked to postpone your Pet Coach sessions until your situation
changes. Remember that, while sessions can be rescheduled, there are no refunds after the
first class.
1) You must be in good physical condition to train a dog. People with back, knee, balance,
breathing or heart problems will not be able to access the building, and those who lack the
physical strength in their hands to control a dog, or who cannot walk briskly for three minutes
at a time, will be unable to perform the training exercises.
2)No children under 12 are allowed at The Pet Coach at any time. This includes the parking lot.
3) You must be over 16 to handle a dog at The Pet Coach
4)From May through December, all dogs who come onto The Pet Coach property must be on a
flea and tick preventative that was dispensed by your veterinarian. This is for the health of
your dog, yourself, and everyone who comes after you. Over-the-counter remedies are not
effective for the tick conditions we have in the mountains. Some examples of acceptable
remedies are : Frontline, Advantix, Comfortis, Premaris, Revolution. Your vet is the final
authority on what will work best for you.
5) Proper attire is essential. You will be working on uneven surfaces both indoors and out. No
sandals, heels or open-backed shoes are allowed in The Pet Coach. If you come to class in
inappropriate footwear, you will not be allowed to participate in training exercises during that
session.
